METHODS: A multicenter retrospective analysis using the ROBUUST (ROBotic surgery for Upper tract Urothelial cancer Study) registry identified patients undergoing surgical treatment for UTUC between 2015-2022 at 12 centers across the United States, Europe, and Asia. Patients were stratified by race and geographic location in which treatment took place. Univariate and multivariate analysis were performed. Recurrence-free survival (RFS) and overall survival (OS) were assessed using Kaplan-Meier analyses. RESULTS: 1446 patients were analyzed. Chemotherapy regimen, pathologic stage, and nodal staging were statistically different across all groups (Table 1). Blacks and Hispanics from the United States had significantly worse RFS relative to Blacks and Hispanics from Europe (p-value=0.0027 and p-value 0.0064, respectively) (Figure 1B and 1C). Asians from the United States had significantly worse RFS relative to Asians from Asia (p-value<0.001) (Figure 1D). On multivariate analysis adjuvant chemotherapy was associated with worse RFS among Blacks (10.21 [1.08, 96.33]; p=.0424) and Asians (HR 2.03 [1.23, 3.34]; p=.0058) within the United States when controlling for age, neoadjuvant and adjuvant chemotherapy, adjuvant bladder instillation and pathologic staging. There were no significant differences in RFS or OS between Whites from the United States and Europe (Figure 1A). CONCLUSIONS: Minority groups from Europe and Asia have better outcomes in regard to RFS following RNU for treatment of UTUC when compared to minority groups from the United States. These differences may be due to socioeconomic factors, treatment accessibility, cultural/ethnic differences in pursing healthcare, and differences in clinical management of UTUC across different continents.
